Submitting a Report

The Office of the State Inspector General (OSIG) relies on citizens and state employees to protect taxpayers from state fraud, waste, and abuse. Please select where you wish to direct your report:

Hotline Unit – State Fraud, Waste, and Abuse Hotline

Please submit your report to the Hotline if you suspect a Virginia executive branch agency or institution of any of the following:

  • Illegal or fraudulent conduct.
  • Waste of funds.
  • Abuse of state property or resources.
  • Gross mismanagement.
  • Gross neglect of duty.

Please note: OSIG’s jurisdiction does not include federal, county, or city governance.

Healthcare Compliance Unit – Complaint Line

Please submit your report to the Healthcare Compliance Unit if you suspect any program or facility operated, licensed, or funded by the Department of Behavioral Health and Developmental Services (DBHDS) has committed any of the following:

  • Abuse.
  • Neglect.
  • Inadequate care.

Ombudsman Unit – Inmate and Family Support Services Line

Please submit your report to the Ombudsman Unit if you have complaints or inquiries about any of the following in a correctional facility operated by the Department of Corrections:

  • Sanitation.
  • Access to proper nutrition and clean and adequate water supplies.
  • Habitable temperatures.
  • Physical or sexual abuse from fellow inmates.
  • Physical or sexual abuse from staff, contractors, or volunteers.
  • Credible threats against self from other inmates, staff, or contractors.
  • Neglect of staff or contractors that results in physical or sexual trauma.
  • Denial or violation of rights.
  • Access to visitation and communication with family and legal representation.
  • Access to medical or mental health care of substance abuse treatment.
  • Access to educational and rehabilitative programming, drug and mental health treatment, and inmate jobs and vocational training.
  • Access to DOC’s administrative remedies process for inmates and their representatives, including the availability of complaint and grievance forms and the timely, unbiased resolution of grievances.
  • Adequate and qualified staffing.
