State Fraud, Waste and Abuse Hotline
As part of government reform initiatives to make Virginia state agencies run more efficiently and effectively, the State Employee Fraud, Waste and Abuse Hotline began operating in October 1992, and since that time has investigated more than 16,000 cases. On October 9, 2012, through Executive Order Number 52 (2012), the State Employee Fraud, Waste and Abuse Hotline was expanded to all citizens of Virginia, not just state employees, and its name was changed to the State Fraud, Waste and Abuse Hotline.
Reports made to the State Fraud, Waste, and Abuse Hotline (Hotline) are anonymous and confidential. You may contact the Hotline by:

Remembering Phil Underwood
The Virginia Office of the State Inspector General (OSIG) mourns the loss of Senior Investigator Phil Underwood, who passed away on Sunday afternoon, Nov. 4, after a months-long struggle with cancer. He was 66.
Phil was one of the founding members of OSIG, having transferred from his role as senior investigator with the Virginia Department of Transportation to serve as a State Fraud, Waste and Abuse Hotline investigator when OSIG was created by an act of the General Assembly in 2012. He served in his role as senior hotline investigator until this past spring, taking leave in April due to his illness.
Over the course of his six years at OSIG, Phil handled and coordinated the investigation of hundreds of hotline complaints, listening with a thoughtful and compassionate ear to those who suspected, witnessed or experienced wrongdoing in the workplace. As a colleague, he was outgoing and sociable, and the first one to make the coffee in OSIG’s kitchen each morning. He was also deeply kind and considerate, always taking interest and moments in his day to ask about the health of an aging parent or the well-being and activities of coworkers’ children.
Our thoughts and prayers are with Phil and his family. He will be deeply missed.
